To download the cremation forms you will need a portable document format (PDF) reader installed on your computer. If you do not have one, you can download the free Adobe Reader.
Once you have downloaded the forms, please print them out and sign the documents where noted by the “X”. The cremation authorization and the denial of embalming must be signed by the legal next of kin. The statement of goods and services may be signed by the person who took care of the charges. (Please keep in mind that this document will only state the base rate for our cremation, not any additional items that you may choose to purchase.)
If your loved one has been remanded to the Dallas or Tarrant County Medical Examiner we will need the next of kin to sign the appropriate agencies release form.

The following document permits competent persons (PRIOR TO DEATH) to appoint an agent to make all decisions about the disposition of their remains after death. Form must be notarized.
